Anatomical Gifts, Driver's License and
Identification Card
The Pennsylvania Department of Transportation is required by law to ask all
driver's license and identification card applicants if they wish to
participate in organ or tissue donation.
Individuals who wish to participate will have "Organ Donor" noted on the
front of the driver's license or identification card. There is no additional
fee for participation or having the "Organ Donor" designation printed.
Declining to participate does not place any additional language on the
license or card.
A separate Anatomical Gift Directive is
not necessary for those who wish to give general authorization for donation
of needed organs and who participate through Pennsylvania Department of
Transportation. Having the "Organ Donor" notation is sufficient to satisfy
all the requirements for consent to donation.
PennDOT also provides form "DL-80" to add or remove the "Organ Donor"
designation with a current driver's license. This is the application for
change, correction, or replacement of a non-commercial driver's license,
