Anatomical Gift Directive, Revoking
Any donor may amend or revoke his or her Anatomical Gift Directive at any time.

If the donor has possession of the only copy of the Anatomical Gift Directive, he or she may simply destroy that document.

If the Anatomical Gift Directive has been delivered to a donee, Bank, or any other storage facility or physician, there are four methods to amend or revoke the document:

1. Deliver an executed statement of amendment or revocation to the donee.
2. Make an oral statement of amendment or revocation in the presence of two witnesses, which must then be communicated to the donee.
3. Make a statement of amendment or revocation to an attending physician during a terminal illness or injury, which is then communicated to the donee.
4. Create a signed statement of amendment or revocation that will be discovered on the donor's person or in the donor's effects.
