Anatomical Gift Directive, Who May Create
Any person who is at least 18 years of age and of sound mind may complete and execute an Anatomical Gift Directive.  A person who is between the ages of 16 and 18 may also execute an Anatomical Gift Directive, provided that parental or guardian consent is noted, by signature, on the same document.
